Coronary Artery Disease Treatment Alternatives

Given the press on specific medical technologies, particularly those with large associated markets, the overall competitive landscape can sometimes be distorted. This is certainly the case with coronary artery stents. The emergence of a multi-billion dollar market for these stents over the past decade, and the emergence of the drug-eluting stent option, first by Johnson & Johnson, then Boston Scientific, seems to suggest that coronary artery disease (CAD) is now and forever shall be dominated by drug-eluting stents. However, despite this apparent trend (since past is not always prologue), their are multiple clinical options available, and emerging, for the treatment of coronary artery disease. In recent posts on the advanced medical technologies blog, we note the treatment options and even the treatment "map" that represents the current and future alternatives for CAD.

To hammer the point, It is worth noting that one radical alternative to stenting and bypass for CAD -- the dyslipidemia treatment by Esperion Therapeutics' ETC-1002, has advanced to Phase I trials this week.

Coronary Stent Sales Globally Driven by Some Big Local Market Differences

Local market drivers account for some big differences in the distribution of coronary stent sales by stent type, and these differences are expected to continue when bioabsorbable stents (and other stent technologies) are introduced to the market over the next few years, according to a global report from MedMarket Diligence on drug-eluting, bare metal and soon-to-emerge bioabsorbable coronary stents, and other anti-restenosis technologies.

Differences in the volume and the growth rates of coronary stent sales stem from some anticipated local market variables, like demographics, and other variables in the unique market dynamics of each geography.

The advance medical technologies blog highlights some of the findings in the MedMarket Diligence report #C245 on coronary stents.

For Now, Drug-Eluting Stents Dominate the Coronary Stent Market

The global market for coronary stents is owned largely by drug-eluting stents (DES), but the size of the patient pool, the drive for lowered cost, ever-increased efficacy and less trauma are spawning the development of multiple new technologies in coronary stents and competitive anti-restenosis technologies. The global market, products, companies, trends and opportunities are the subject of the 2009 report, "Worldwide Coronary Stents 2009," from MedMarket Diligence.

Currently, drug-eluting stents (DES) command 60% of typical markets for coronary stents and, in some markets like China, the share is much higher. Growth in the market for DES, at over 7%, is more than twice what it is for bare metal stents, since DES have demonstrated, through anti-restenosis, the ability to considerably reduced the need for repeat revascularization. The market for coronary stents will therefore continue to grow, penetrating deeper into bare metal stent markets and furthering the erosion of demand for the more invasive surgical alternative, coronary artery bypass grafting.

"The number of active companies and the size and growth of the market for coronary stents speaks to the intensity of development," says Patrick Driscoll of MedMarket Diligence, publisher of the report, 'Worldwide Coronary Stents Market, 2009'. At least 75 companies are active in this arena, with current products on the market or at various stages of development. Few avenues are being overlook in development, from stent material types, stent surface technology, drug-coating alternatives, drug-coating processes and others.

Development of stent alternatives also includes bioabsorbable stents, which may be the likely successor to drug-eluting stents if they achieve the goal of providing revascularization with markedly better long-term clinical outcomes. Bioabsorbales under development may well enter some markets as soon as two years from now.

Moreover, the premise of drug-eluting stents is being challenged through alternatives like drug-eluting balloon angioplasty and other device and non-device alternatives.

The MedMarket Diligence report #C245 is described at http://www.mediligence.com/rpt/rpt-c245.htm and details the worldwide market for the use of alternative stent devices in the treatment of coronary artery disease. The report assesses the treatment of coronary artery disease by alternative treatment methodologies (percutaneous intervention with or without stenting, coronary artery bypass grafting alternatives and other medical/surgical treatment options), with estimated current/forecast caseload across treatment types.

The report provides full year (actual) 2008 market size and share data, with forecast market data to 2017, for the U.S., Europe, Asia/Pacific and Rest of World.

The report details currently approved coronary stents, providing data on current and forecast impact on the market. The report also details the current state of product development in bare stents, drug-coated or drug-eluting stents and bioabsorbable stents in coronary applications and details the products under development, the anticipated timeline and impact of their market introduction. The report assesses the current and ten-year forecast global market for coronary stents, with segmentation by U.S., Europe, Asia/Pacific and Rest of World. The report provides competitor market shares by these geographic regions. The leading and key emerging coronary stent manufacturers are profiled with current product offerings, development status and market strengths/advantages.

Competition Remains High in Coronary Stent Market, According to New Global Market Report

The global coronary stent market faces continued growth, development and competition from a wide range of technology types and competitors. The market is detailed in a new report from MedMarket Diligence.

Since the advent of coronary stents, followed by the emergence of drug eluting stents, the field of stents in the treatment of coronary artery disease has been characterized by aggressive development of new technologies, from minor changes in strut design and drug-coating processes to more radical thinking of the kind that lead from bare metal stents to drug-eluting stents. The coronary stent market is currently in a phase in which the clinical and market success of drug-eluting stents introduced six years ago has stimulated innovation toward emergence of new drug-eluting stent types from many competitors. While the less invasiveness of coronary stents (delivered percutaneously) and outcomes of drug-eluting stents that are nearly comparable to coronary bypass are enabling coronary stents to expand the absolute market of coronary stents by capturing caseload from CABG, the market once dominated by Johnson & Johnson's Cordis division is now becoming progressively fragmented with new competitors. The market is now facing new challenges with the pending emergence of bioabsorbable stents, drug-eluting balloons and other competitive alternatives to the established stent market.

The global market for coronary stents, examining the products, technologies, companies and worldwide markets for bare metal, drug-eluting and other coronary stents, is the subject of MedMarket Diligence's report #C245, "Worldwide Coronary Stents 2008-2017" (published May 2009). See link.

"The nature of coronary artery disease, as a major source of morbidity and mortality worldwide, and the support (including especially reimbursement) that treatments receive in the healthcare arena, is a major impetus for technology development," says Patrick Driscoll of MedMarket Diligence. According to Driscoll, there are no fewer than 75 active companies developing and marketing coronary stents and competitive technologies, and the market is likely to become even more consolidated unless new, innovative anti-restenosis technologies are introduced that demonstrate profound clinical advantages over existing options.

The market is facing the emergence, in particular, of bioabsorbable stents, which have demonstrated their potential to provide the necessary anti-restenotic benefit following percutaneous intervention (e.g., angioplasty), then "simply" dissolve and be absorbed at six months or later. Such a device has the potential to eliminated longer term biocompatibility and otherwise inflammatory risk associated with permanently implanted stents. Bioabsorbable stents, too, may be bare or, more likely, containing drug coatings.

The MedMarket Diligence report details the worldwide market for the use of alternative stent devices (cardiac stents or heart stents) in the treatment of coronary artery disease. The report assesses the treatment of coronary artery disease by alternative treatment methodologies (percutaneous intervention with or without stenting, coronary artery bypass grafting alternatives and other medical/surgical treatment options), with estimated current/forecast caseload across treatment types.

The report is described at this link. The report may be ordered for immediate download online.

Coronary stents and variable geographic adoption

Coronary stents have demonstrated a uniquely different pattern of adoption in different parts of the world. The typical differences between the U.S., Europe and other geographies in terms of relative share of the market, do not apply for coronary stents. In particular is the case of China.

Coronary Stents: Emerging New Stent Alternatives

Bare metal stents and drug-eluting stents (DES) are well established. Industry participants are also well along toward developing bioabsorbable stents. But there remain a number of unique stent technologies in advanced stages of development, one or more of which have excellent potential to impact existing markets.

Pericardium covered

Radioactive stents

Biomimetic coatings

Hydrophilic coatings

Titanium-nitride-oxygen coatings

Genetically-engineered bioactive coating

Endothelial cell-stimulating coatings

Synthetic graft stent alternative

Nano-polymer coatings

Stent with embolic protection

While drug-eluting stents have gained solid and growing shares of the worldwide stent market, interventional cardiologists and healthcare systems have demonstrated considerable propensity to shift toward or away from different stent alternatives based on available data --- even incomplete data, which was clearly the case with late-stage thrombosis putting a temporary pinch on DES sales (see also recent data on Xtent's Custom NX that may just pull Xtent back from the brink of shutting down). Aggressive innovators in stent technologies may be rewarded with substantial caseload if their designs can demonstrate even nominal improvements over available stents, a point embraced as readily by the market leaders as by one-device companies.

Anti-Restenosis Devices Come in Many Forms

In the treatment of coronary artery disease (CAD), the option with the highest volume and growth is angioplasty and stenting, as a result of demonstrated outcomes and the all-important minimal invasiveness of percutaneous procedures (compared to coronary artery bypass). The market for CAD treatments has rapidly evolved and will continue to evolve as long as there is incidence of coronary artery disease. Current trends in that evolution are focused not on the manner of revascularization, which is predominantly by angioplasty, but on the manner of maintaining vessel patency by preventing restenosis. Here, the market will evolve across a variety of stent and other device options.

Cardiology/Cardiovascular Device Technologies Drive Medtech Startups

The field of cardiology and cardiovascular medicine drives the formation of many new medtech startups. The nature of clinical practice in cardiology, addressing diseases and disorders of the heart and vasculature, which are in many cases life-threatening and have high prevalence worldwide, is ideal for the development of devices in coronary artery disease, peripheral vascular disease, arrhythmia, heart failure, embolic protection/stroke and many others.

A review of companies formed in recent years and captured in the Medtech Startups Database illustrates that the cardiology/cardiovascular arena is among the most prolific in medtech startup formation. In less than five years, over one hundred (100) companies have been founded with a focus on either cardiology diagnostics or therapeutics.

Interventional cardiology in particular, including the clinical procedures of angioplasty, stenting and related procedures, have contributed to markets with an aggregate value near $10 billion worldwide.

The Medtech Startups Database is an online subscription database of medical device and competing or complementary technology companies, with details on company area(s) of focus, products/technology status, principals and other contact details. The database is described here.

In the field of cardiology, MedMarket Diligence has recently published (May 2009) report #C245, "Worldwide Coronary Stents Market, 2008-2017".
Least invasive (totally endoscopic) coronary artery bypass and other points on the coronary artery disease treatment timeline

The developmental timeline goes something like this:

Coronary artery bypass is developed, dramatically increasing quality of life for a coronary artery disease, where no real alternative existed previously.

Percutaneous transluminal coronary angioplasty is developed, providing a less invasive means to provide reperfusion of the myocardium, but needs to be repeated occasionally due to restenosis and mitigating the "less invasive" quality of PTCA.

Stents are developed to maintain patency following angioplasty, reducing but not eliminated the need for repeat procedures

Drug-eluting stents (DES) are developed that provide anti-restenosis and maintain patency and again reduce the need for repeat procedures

Occurrence of late stage thrombosis associated with DES emerges, causing a momentary lull in stent sales, but proving temporary, or at least limited to a small cohort.

Bioabsorbable stents are under development to improve DES outcomes

Other developments are in progress (drug-eluting balloons, HDL therapy, gene therapy cell therapy)

"Less invasive" (MIDCAB) and "least invasive" (TECAB) options for coronary bypass are under development to remove the invasiveness of bypass and make it more competitive with percutaneous procedures.

This timeline has many more future chapters on it, but what follows the development of minimally invasive direct coronary artery bypass (MIDCAB) or totally endoscopic coronary artery bypass (TECAB ) is highly unlikely to be the demise of drug-eluting stents.

A post on the advanced medical technologies blog provides a description and brief video on TECAB. .

Taking the lead in the coronary stent market: U.S.? Europe? China?

For many medical technology markets, the dynamics of technology development are usually such that innovation arises in western economies (especially U.S. and Europe) and migrates to Asia/Pacific and the rest of the world.

But the dynamic is different in the market for coronary stents. And it has much less to do with technology migration than market size.

Coronary Stents Market Worldwide Holds Big Incentive for Growth, Opportunity in Novel Technologies, According to New MedMarket Diligence Report

The market for coronary stents is, in many ways, still emerging. While coronary stents have been on the commercial market in one form or another for several years, the technologies and materials used to create the devices are improving every day. In addition, innovative technology is bringing new classes of devices (e.g., fully degradable stents) to market, technologies that grow the market and even expand stent therapy to new patient populations in some cases.
