Advanced wound care products accelerating healing and market, according to new MedMarket Diligence report

Traditional wound management is comprised of technologies that do little to accelerate wound healing. Given chronic wound costs, advanced technologies are now being rapidly adopted to change this, according to a new MedMarket Diligence report.

The general wound management market is a mature established business sector including both long-established product groups and more recently evolved approaches to wound management. The main components of the market may be summarized as follows:

Traditional wound care products. These include bandages, dressings and swabs made of generic cotton and textile fabrics.

Wound closures. This sector includes long-established devices including sutures and staples, as well as newly developed methods of wound closure such as tissue sealants and glues; also haemostatic devices to help arrest bleeding while closing the wound. These are mainly used in the surgical environment.

Advanced wound management products. These include dressings based on moist wound healing concepts (made of, biopolymers, hydrocolloids, composites, foam materials, films, etc). These are used mainly for chronic and slow-to-heal wounds. The sector also includes so-called active therapies including tissue engineered products, biomaterial composites, and new physical modes of activating wound healing, the latter having made major inroads into the marketplace within the past 2-3 years. Newly emerging classes of products in this sector include growth factors and angiogenesis promoters.

Consumer wound care. This sector includes a wide range of wound care products, mainly designed for wound closure and protection, and mostly for use in a first aid context.

Historically, wounds were treated with absorbent coverings that were designed to absorb the high levels of exudates produced from surgical, traumatic, or chronic wounds. These dressings delivered the benefit that they aesthetically covered up the sight of the wound, offered some absorptive capacity, and protected the wounds from getting dirty, offering some protection from infectious agents.

The ideal wound dressing achieves a number of goals. It should cover the wound and control odor; be conformable, mimicking the elasticity and stretch characteristics of skin, and yet easy to apply to the wound. It is now recognized that ideal wound dressings should control the fluid level at the wound surface in moist wound healing conditions, while absorbing excess liquid exudates. Wound dressings should also exclude infectious agents, and disinfect pathological levels of infectious agents. These dressings should require to be changed as infrequently as possible without detriment to the wound healing process, and acceleration of wound healing without scar formation is an elusive goal. Clearly a number of wound dressings with different attributes are required to address the wide variety of wound types, symptoms and the variability within any single wound.

Advanced wound care products are growing in use due to their ability to accelerate wound healing, a benefit readily perceived by physicians and healthcare providers for its better outcome and reduction in cost of care. Select advanced wound care products are growing at or near double digit rates, including hydrocolloid dressings, physical wound therapies (e.g., negative pressure) and growth factors.

Hemostats market: dominant players, but many competitors

The market for hemostats in the management of bleeding for traumatic and surgical wounds has a long history, spanning over a hundred years in clinical practice. Although some key players control a large aggregate share of the market, many competitors are working on formulations of gelatins, collagens, fibrin-based products and others in an effort to improve clinical practice and/or costs -- and gain market share. The low barriers to entry in this arena (particularly compared to surgical sealants and glues) and the size of the current and potential caseload continue to be incentives for hopeful market participants.

The "advanced medical technologies" blog highlights the worldwide market for hemostats, drawn from the MedMarket Diligence report #S175, "Worldwide Surgical Sealants, Glues, Wound Closure and Anti-Adhesion, 2009-2013."

Neurological applications of surgical sealants, closure and hemostasis

One of the key areas of interest for development of products in surgical sealing, closure and hemostasis is in neural tissue surgery, encompassing chronic stroke, spinal cord trauma, neurovascular defects, and brain tumor treatments. In the U.S. alone, there were some 5.6 million patients in this category in 2008 who could benefit from these products.

The "advanced medical technologies" blog reviews the potential applications and prevalence of caseload for neurological use of surgical sealants, closure and hemostasis.

Data and analysis was drawn from Report #S175, "Worldwide Market for Sealants, Glues and Wound Closure, 2009-2013."

MedMarket Diligence's global analysis on the market and opportunities for products in surgical securement (surgical sealants, glues and other wound closure, hemostasis, and anti-adhesion) is described, with complete table of contents and list of exhibits at this link.

Trends in the management of surgical wounds

Surgical wounds account for the vast majority of skin injuries. We estimate that there are over 100 million surgical incisions a year, which require some wound management treatment. Approximately 80% of these wounds use some form of closure product (sutures, staples, and tapes). Many employ hemostasis products, and use fabric bandages and surgical dressings.

Surgical wounds are projected to increase in number at an annual rate of 3.1%, but overall the severity and size of surgical wounds will continue to decrease over the next ten years as a result of the continuing trend toward minimally invasive surgery.

Surgical procedures generate a preponderance of acute wounds with uneventful healing and a lower number of chronic wounds, such as those generated by wound dehiscence or post-operative infection. Surgical wounds are most often closed by primary intention, using products such as sutures, staples, or glues, where the two sides across the incision line are brought close and mechanically held together. Surgical wounds that involve substantial tissue loss or may be infected are allowed to heal by secondary intention where the wound is left open under dressings and allowed to fill by granulation and close by epithelialization. Some surgical wounds may be closed through delayed primary intention where they are left open until such time as it is felt it is safe to suture or glue the wound closed.

A significant feature of all wounds is the likelihood of pathological infection occurring. Surgical wounds are no exception, and average levels of infection of surgical wounds are 7%-10% dependent on the procedure. These infections can be prevented by appropriate cleanliness, surgical discipline and skill, wound care therapy, and antibiotic prophylaxis. Infections usually lead to more extensive wound care time, the use of more expensive products and drugs, significantly increased therapist time, and increased morbidity and rehabilitation time. A large number of wounds will also be sutured to accelerate closure, and a proportion of these will undergo dehiscence and require aftercare for healing to occur.

Structure of the wound management products market

The general wound management market is a mature established business sector including both long-established product groups and more recently-evolved approaches to wound management.

Advanced wound care products. These include dressings based on moist wound healing concepts (made of, biopolymers, hydrocolloids, composites, foam materials, films etc). These are used mainly for chronic and slow-to-heal wounds. The sector also includes so-called active therapies including tissue engineered products, biomaterial composites, and new physical modes of activating wound healing, the latter having made major inroads into the marketlace within the past 2-3 years. Newly emerging classes of products in this sector include growth factors and angiogenesis promoters.

Consumer wound care. This sector includes a wide range of wound care products, mainly designed for wound closure and protection, and mostly for use in a first aid context.

Many major wound management businesses provide products within all the categories above. They source these through their own proprietary manufacturing capabilities, and where technologies are old and generic, companies source them from specialist wound dressing manufacturers. Within the categories of active therapies, wound management businesses may have to partner with specialist developers of these new technologies, to pay a premium for the uniqueness of their technologies, and often to acquire the technologies or companies in order to optimize manufacture and supply.

MedMarket Diligence's pending (September 2009) report, "Worldwide Wound Management, 2008-2017: Established and Emerging Products, Technologies and Markets in the U.S., Europe, Japan and Rest of World," assesses the current and forecast market, and market opportunities, for established and emerging products and technologies in wound management.

Approved neurosurgery indications for high-strength surgical glues in Europe

High-strength glue products were approved for topical closure applications in the United States in the late 1990s. Outside the United States, high-strength glues and adhesives have been approved in some countries for a number of years and have built up a track record of use in internal surgery. With time, high-strength glues will come to be used in a wider range of procedures and enable new procedures to be performed more economically with better outcomes and reduced morbidity and mortality.

We have reviewed applications and indications of commercial products and include a list of approved and indicated neurosurgery procedures in the European Union, as it illustrates the growing potential for these high-strength glues once they gain universal approval and adoption within the surgical community worldwide.

(See the complete list of procedures in Europe for high strength glues, encompassing the specific procedures in cardiac surgery, digestive tract endoscopy, general surgery, gynecological surgery, urological surgery, interventional radiology and vascular neuroradiology, pediatric surgery, and ENT surgery in Report #S175.)

MedMarket Diligence's global analysis on the market and opportunities for products in surgical securement (surgical sealants, glues, other wound closure, hemostasis, and anti-adhesion) is described, with complete table of contents and list of exhibts at this link.

Growth in surgical procedures and new techniques driving markets for surgical closure and securement

Major surgical procedures, as in cardiovascular and spine surgery, continue to evolve to reduce invasiveness, minimize complications and improve clinical outcomes. Caseloads remain persistently high in many surgical applications by virtue of the acute nature of the conditions, the clinical benefits derived from surgery, and the demographics of the aging population.

The advanced medical technologies blog explores this underlying set of drivers for the use of advanced surgical closure and securement products, including surgical sealants, glues, hemostasis and other wound management products. These products are continuing to penetrate traditional products for management of surgical and traumatic wounds.

High-strength surgical glues: From the outside looking in

Use of high-strength glues in topical applications is common and growing, despite the fact that high-strength glues are not as strong as sutures. Their ease of use, ability to reduce or eliminate scarring and, probably most importantly, their effective marketing by manufacturers are all factors leading to their strong historic, and forecast, market growth. However, challenges remain for the application of high-strength glues for internal applications, given the need for better adhesion and the problem of toxicity of existing glues, which are predominantly cyanoacrylate-based.

The advanced medical technologies blog highlights high strength glues and their opportunities and challenges, with data drawn from the MedMarket Diligence report #S175, "Worldwide Market for Surgical Sealants, Glues and Wound Closure, 2009-2013."

Sutures (slowly but surely) giving way to cyanoacrylate-based and other high-strength medical adhesives

Cyanoacrylate products are the main form of high-strength surgical glue that is approved for human clinical use in the worldwide market. However, as strong in adhesion as these products are, they remain less strong than sutures. They also have the distinct problem of being toxic, so for now their use is almost exclusively limited to external applications. However, about a dozen companies are developing other high-strength adhesives that will get past these challenges. As a result, high-strength glues will progressively take over turf held by sutures.

Surgical, traumatic and other wound type prevalence worldwide; targets for surgical securement products

Wound types and the applicability of surgical securement products in their treatment are discussed in the MedMarket Diligence report #S175, "Worldwide Surgical Sealants, Glues and Wound Closure, 2009-2013."

A useful starting point for determining which adjunctive surgical closure and securement products are appropriate for different wound healing treatments is to recognize the major distinction between different wound types. Surgical wounds are usually appropriate for treatment with adjunctive surgical closure and securement products because they are created under clean conditions, the usual acute healing cascade of events begins immediately and control of the bleeding and closure process can lead to accelerated healing, improved prognosis, and enhanced aesthetic effects such as reduced scarring.

Surgical wounds offer the potential for devices to ensure hemostasis, prevent internal adhesions and anastomoses, soft tissue securement, and closure of the skin. Traumatic wounds also offer potential for skin closure products and for hemostats, and adhesion prevention during post-trauma surgery. New wound-covering sealant products may also offer potential for treatment of cuts, grazes, and burns.

Chronic wounds are generally not amenable for treatment by adhesives, sealants and hemostats unless the wound has been debrided to a sterile bleeding surface (in which case it becomes like a surgical wound), or the product offers some stimulant activity; many hemostats exhibit some inflammatory and cytokinetic activity, which has been associated with accelerated healing.

To help determine how these applications for closure and securement products may translate into market potential, it is relevant to note the prevalence as well as the growth rates for each of the different types of wounds; The exhibit below illustrates this.

Growth in patient caseload for surgical securement products

Caseload continues to grow for the use of the broad category of products called "surgical securement", encompassing advanced products in surgical sealants, glues, hemostasis, and other wound closure and anti-adhesion products, according to a 2009 report by MedMarket Diligence, LLC.

"Worldwide, over 100 million surgical and traumatic wound types and applications are potential candidates for use of these products, and the breadth of these applications illustrate the great clinical utility of these products," says Patrick Driscoll of MedMarket Diligence. "These applications span cardiovascular, orthopedic and arthroscopic, digestive, urological, neurological, cosmetic, and general surgery, displacing many traditional wound closure products."

The market for surgical closure and securement has entered a phase in which major driving forces are the introduction of new procedures and techniques by the surgical profession, the development by the medical device industry of new wound closure devices and biomaterials, and the growing willingness of surgical specialists to use these devices in appropriate circumstances. There is now a continuum between simple closure using sutures and the use of specially designed devices and delivery systems with new bioresorbable securement materials either as supplements to conventional closure methodology or as stand-alone replacements.

Among all potential clinical applications, surgical securement products play critical roles in reducing mortality and complications for roughly 26 million surgical procedures and, in another 50 million surgical procedures, securement products provide distinct improvements in clinical outcomes. In another 40 million procedures, securement product reduce procedure time, provide cost savings or provide other benefits.

The MedMarket Diligence report #S175, "Worldwide Surgical Sealants, Glues, Wound Closure and Anti-Adhesion Markets, 2009-2013," is a market and technology assessment and forecast of products in wound closure. The report details the current and emerging products, technologies and markets involved in wound closure and sealing using sutures and staples, tapes, hemostats, fibrin and sealant products, medical adhesives and products to prevent post-surgical adhesions. The report provides a worldwide current and annual forecast to 2013 of the markets for these technologies, with particular emphasis on the market impact of new technologies through the coming decade. The report provides specific forecasts and shares of the worldwide market by segment for the U.S., Europe (United Kingdom, German, France, Italy, BeNeLux), Latin America, Japan and Rest of World.

The report provides background data on the surgical, disease and traumatic wound patient populations targeted by current technologies and those under development, and the current clinical practices in the management of these patients, including the dynamics among the various clinical specialties or subspecialties vying for patient population and facilitating or limiting the growth of technologies.

The report establishes the current worldwide market size for major technology segments as a baseline for and projecting growth in the market over a ten-year forecast. The report also assesses and projects the composition of the market as technologies gain or lose relative market performance over this period.

Adoption of surgical securement products by clinical category and key determinants

The potential for products in the surgical securement (sealants, glues, sutures/staples, hemostats, anti-adhesion) is driven by clinical category and the impact of the particular products, in each specialty, on a hierarchy of determinants, from the "important and enabling" to the "aesthetic and perceived benefits". This results in a variable distribution by clinical category (see blog entry for category definitions) in the potential of each.

Dermal repair disorders, prevalence and applications for securement products

Skin securement has always been an essential final step in surgical procedures. Historically, the skin surface was sutured; in recent years a number of advances have been made, including new tapes, sutures, staples, hemostats, and glues. High strength glues, in particular, find some of their biggest potential in dermal applications, given the problems of, for example, cyanoacrylate toxicity for internal applications.

The advanced medical technologies blog, drawing on MedMarket Diligence report #S175 on the Worldwide Market for Surgical Sealants, Glues and Wound Closure," illustrates the potential for surgical securement products for their application in treatment of venous ulcers, pressure ulcers, and burns, as well as for their significant volume of usage in plastic surgery.

Securement products worldwide are described in the MedMarket Diligence report #S175.
